高频通气在小儿支气管异物取出术中的应用

摘要

目的 探讨深镇静麻醉下普通麻醉机高频通气在小儿气管支气管异物取出术的应用。方法 回顾小儿支气管镜下气管支气管异物取出术30例,年龄2~4岁。研究组(M组)采用深镇静的麻醉方式,对照组(P组)采用常规麻醉方式。供氧方式硬质支气管镜插入气管后接Dräger麻醉机行高频通气。监测患儿生命体征,记录患儿SPO2,手术医生满意度评分及并发症情况。结果 30例均成功取出异物,无一例死亡及严重的术后并发症;M组手术医生满意度评分值高于P组(P<0.05)。术中屏气或呛咳例数、呼吸暂停例数、SpO2下降例数及体动例数M组均低于P组(P<0.05)。结论 深镇静麻醉下,应用普通麻醉机高频通气,对患儿生理干扰小,清醒快,提高患者安全性。

Abstract

Objective: To explore the application of high-frequency ventilation of general anesthesia machine in the removal of tracheobronchial foreign bodies in children under deep sedation anesthesia.
Methods: Thirty cases of tracheobronchial foreign body removal under bronchoscopy in children, aged 2-4 years, were retrospectively reviewed. The study group (M group) used deep sedation anesthesia, while the control group (P group) used conventional anesthesia. Oxygen supply: A rigid bronchoscope was inserted into the trachea and then connected to a Dräger anesthesia machine for high-frequency ventilation. The vital signs of the children were monitored, the SPO2 of the children, the satisfaction score of the surgeon and the complications were recorded.
Results: The foreign bodies were successfully removed in all 30 cases, and there was no death or serious postoperative complications; the satisfaction score of surgeons in group M was higher than that in group P (P<0.05). The number of breath-holding or coughing cases, apnea cases, SpO2 decrease cases and body movement cases in group M were lower than those in group P (P<0.05).
Conclusion   Under the depth of the depression, the use of ordinary anesthesia machines high-frequency ventilation, the physiological interference in children is small, clear, and improve patient safety.
