Inhibitory and pro-apoptotic effects of tranexamic acid injection on hypertrophic scar and keloid fibroblasts: A clinical study of 59 cases
59例氨甲环酸(传明酸)注射液抑制增生性瘢痕与瘢痕疙瘩成纤维细胞增殖并诱导细胞凋亡的作用临床研究

摘要

目的 观察氨甲环酸(传明酸)注射液皮损内注射抑制增生性瘢痕与瘢痕疙瘩的临床疗效及安全性,为临床治疗提供参考依据。方法 回顾性分析2022年1月至2023年12月本院皮肤美容科收治的59例增生性瘢痕与瘢痕疙瘩患者的临床资料,所有患者均接受氨甲环酸注射液皮损内注射治疗,治疗周期为12周。采用温哥华瘢痕量表(VSS)评估治疗前后瘢痕的色泽、血管分布、厚度及柔软度,同时记录患者的瘙痒、疼痛症状改善情况及不良反应发生情况。结果 59例患者均完成12周治疗及随访,总有效率为86.44%(51/59)。治疗后患者VSS总评分及色泽、血管分布、厚度、柔软度各分项评分均显著低于治疗前(P<0.05);瘙痒、疼痛症状缓解率分别为90.74%(49/54)和88.46%(46/52)。治疗期间仅6例患者出现轻微不良反应,发生率为10.17%,均无需特殊处理可自行缓解。结论 氨甲环酸注射液皮损内注射治疗增生性瘢痕与瘢痕疙瘩疗效确切,能显著改善瘢痕外观及症状,且安全性高,值得临床推广应用。

Abstract

Objective To observe the clinical efficacy and safety of intralesional injection of tranexamic acid injection in the treatment of hypertrophic scars and keloids, and to provide a reference for clinical treatment.
Methods A retrospective analysis was conducted on the clinical data of 59 patients with hypertrophic scars and keloids admitted to the Department of Dermatology and Cosmetology of our hospital from January 2022 to December 2023. All patients received intralesional injection of tranexamic acid injection for a treatment course of 12 weeks. The Vancouver Scar Scale (VSS) was used to evaluate the color, vascularity, thickness and pliability of scars before and after treatment. Meanwhile, the improvement of pruritus and pain symptoms and the occurrence of adverse reactions were recorded.
Results All 59 patients completed the 12-week treatment and follow-up, with a total effective rate of 86.44% (51/59). After treatment, the total VSS score and the scores of color, vascularity, thickness and pliability were significantly lower than those before treatment (P<0.05). The remission rates of pruritus and pain were 90.74% (49/54) and 88.46% (46/52), respectively. Only 6 patients had mild adverse reactions during the treatment, with an incidence rate of 10.17%, all of which resolved spontaneously without special treatment.
Conclusion   Intralesional injection of tranexamic acid injection has definite efficacy in the treatment of hypertrophic scars and keloids. It can significantly improve the appearance and symptoms of scars with high safety, and is worthy of clinical promotion and application.
