鞘膜内睾丸扭转CT及MRI影像表现

摘要

目的 探讨鞘膜内睾丸扭转的计算机断层扫描(CT)及磁共振成像(MRI)表现,提高对本病的认识及早期诊断。方法 收集并分析23例经手术证实的睾丸扭转患者的临床及影像资料,总结该病临床影像学特征。结果 23例患者中,19例位于左侧,显著多于右侧(p<0.001),无诱因突发睾丸疼痛并进行性加剧为主要表现。23例均表现为睾丸及附睾体积增大、位置抬高,CT及MRI可见不同程度的精索增粗、扭曲伴鞘膜积液,睾丸强化明显减退或无强化;17例中晚期扭转睾丸多见大片状、多发条纹状坏死,易发生睾丸萎缩。结论 CT及MRI能敏感显示睾丸组织活性程度,可为早期诊断和治疗提供重要依据。

Abstract

Objective To explore the Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) manifestations of intratesticular torsion, and to enhance the understanding and early diagnosis of this disease.
Methods The clinical and imaging data of 23 patients with confirmed testicular torsion by surgery were collected and analyzed. The clinical imaging characteristics of this disease were summarized.
Results Among the 23 patients, 19 were on the left side, significantly more than on the right side (p < 0.001). The main manifestation was sudden and progressively increasing pain in the testicle without any trigger. All 23 cases showed increased volume and elevated position of the testicle and epididymis, and CT and MRI showed varying degrees of testicular cord thickening and twisting accompanied by hydrocele of the tunica vaginalis, with significant reduction or absence of testicular enhancement; 17 cases of advanced torsion of the testicle were mostly large patchy and multiple streak-like necrosis, and were prone to testicular atrophy.
Conclusion   CT and MRI can sensitively display the degree of testicular tissue activity and provide important basis for early diagnosis and treatment.
