eCASH策略联合沉浸式虚拟现实技术在重症烧伤患者镇痛镇静管理中的应用效果研究

摘要

目的 探讨ICU早期舒适‑充分镇痛‑最小镇静‑最大化人文关怀理念(early Comfort using Analgesia, minimal Sedatives and maximal Humane care, eCASH)策略联合沉浸式虚拟现实(VR)技术在重症烧伤患者镇痛镇静管理中的效果。方法 将60例重症烧伤患者随机分为对照组和观察组,各30例。对照组实施eCASH镇痛镇静管理,观察组联合沉浸式VR干预,比较两组干预效果。结果 观察组镇静达标、停药后清醒及烧伤ICU时间更短,镇痛镇静药物用量、24 h镇痛次数、疼痛及焦虑评分、不良事件发生率更低,躁动-镇静评分量表(Richmond Agitation-Sedation Scale, RASS)评分、心率和呼吸频率更平稳,满意度更高(P<0.05)。结论 eCASH策略联合沉浸式VR技术可改善重症烧伤患者镇痛镇静效果,减少药物用量及不良事件,提高满意度。

Abstract

Objective To investigate the effect of theearly Comfort using Analgesia, minimal Sedatives and maximal Humane care (eCASH) strategy combined with immersive virtual reality (VR) technology in the analgesic and sedative management of severe burn patients.
Methods Sixty severe burn patients were randomly divided into a control group and an observation group, with 30 cases each. The control group received eCASH analgesic and sedative management, while the observation group underwent combined immersive VR intervention. The intervention effects of both groups were compared.
Results The observation group had shorter durations for achieving target sedation, post-discontinuation awakening, and burn ICU stay, lower doses of analgesic and sedative drugs, fewer 24-hour analgesic administrations, lower pain and anxiety scores, and reduced incidence of adverse events. Additionally, the Richmond Agitation-Sedation Scale (RASS) score, heart rate, and respiratory rate were more stable, and satisfaction was higher in the observation group (P < 0.05).
Conclusion   The eCASH strategy combined with immersive VR technology can improve analgesic and sedative outcomes in severe burn patients, reduce drug dosage and adverse events, and enhance satisfaction.
