Household ozone inhalation inducing acute respiratory distress syndrome: A case report
家用臭氧中毒致急性呼吸窘迫综合征:西维来司他钠和肺保护策略 成功治疗1例

摘要

目的 探讨家庭臭氧吸入致急性呼吸窘迫综合征(acute respiratory distress syndrome, ARDS)的临床特征及救治要点,评估其治疗策略的有效性。方法 回顾性总结1例因等离子燃气灶故障吸入臭氧后发生重度ARDS患者的诊疗过程,详细分析其发病诱因、临床表现、影像学特点、鉴别诊断和治疗方法。结果 经鼻高流量氧疗、糖皮质激素基础上,于病程第3天加用中性粒细胞弹性蛋白酶抑制剂西维来司他钠。治疗后炎症指标迅速下降,氧合及影像学显著改善,患者康复出院,干预手段及时、有效且符合当前临床指南推荐标准。结论 家庭臭氧吸入可致严重ARDS,易被忽视。早期识别、综合支持并靶向抑制中性粒细胞介导的损伤是救治关键。本例为类似中毒事件提供了诊疗借鉴。

Abstract

Objective To investigate the clinical characteristics and key treatment strategies for acute respiratory distress syndrome (ARDS) induced by household ozone inhalation, and to evaluate the effectiveness of the therapeutic approach.
Methods A retrospective analysis was conducted on the diagnosis and treatment process of a patient with severe ARDS caused by ozone inhalation due to a malfunctioning plasma gas stove. The study detailed the triggers, clinical manifestations, imaging characteristics, differential diagnosis, and treatment methods.
Results Based on high-flow nasal cannula oxygen therapy and corticosteroids, the neutrophil elastase inhibitor sivelestat sodium was added on day 3 of the clinical course. Following treatment, inflammatory markers decreased rapidly, and both oxygenation and imaging findings improved significantly. The patient recovered and was discharged. The interventions were timely and effective, aligning with current clinical guideline recommendations.
Conclusion   Household ozone inhalation can lead to severe ARDS, which is often overlooked. Early detection, comprehensive supportive care, and targeted inhibition of neutrophil-mediated injury are critical for successful treatment. This case provides a reference for the diagnosis and management of similar poisoning incidents.
