Analysis of the impact of preserved amniotic membrane transplantation in the treatment of moderate to severe ocular chemical burns and acute phase of thermal burns
羊膜移植治疗中重度眼化学烧伤和热烧伤急性期的影响分析

摘要

目的 探讨羊膜移植联合肝素治疗中重度眼化学烧伤和热烧伤急性期的临床效果。方法 选取2022年7月—2024年5月收治的66例中重度眼化学烧伤/热烧伤急性期患者，分为观察组（33例，40眼，羊膜移植+常规治疗+肝素）与对照组（33例，40眼，常规治疗），比较眼表愈合时间、角膜透明度及并发症发生率。结果 观察组眼表愈合时间（18.25±3.12）d，短于对照组（26.78±4.56）d（t=10.234，P=0.000）；治疗1个月角膜透明度评分（2.15±0.67）分，高于对照组（1.32±0.54）分（t=6.897，P=0.000）；并发症发生率7.50%，低于对照组25.00%（χ²=5.165，P=0.023）。结论 羊膜移植联合肝素可加速急性期眼烧伤愈合，改善角膜透明度，降低并发症风险，值得临床推广。

Abstract

Objective To investigate the clinical efficacy of amniotic membrane transplantation combined with heparin in the treatment of moderate to severe ocular chemical burns and acute phase of thermal burns.
Methods Eighty patients with moderate to severe ocular chemical burns or acute phase of thermal burns admitted from July 2022 to May 2024 were selected. They were divided into the observation group (33 cases, 40 eyes, treated with amniotic membrane transplantation + conventional treatment + heparin) and the control group (33cases, 40 eyes, treated with conventional treatment only). The ocular surface healing time, corneal transparency, and complication rate were compared between the two groups.
Results The ocular surface healing time of the observation group was (18.25±3.12) days, which was shorter than that of the control group [(26.78±4.56) days] (t=10.234, P=0.000). After 1 month of treatment, the corneal transparency score of the observation group was (2.15±0.67) points, which was higher than that of the control group [(1.32±0.54) points] (t=6.897, P=0.000). The complication rate of the observation group was 7.50%, which was lower than that of the control group (25.00%) (χ²=5.165, P=0.023).
Conclusion   Amniotic membrane transplantation combined with heparin can accelerate the healing of ocular burns in the acute phase, improve corneal transparency, and reduce the risk of complications, which is worthy of clinical promotion.
