Analysis of the effect of the focus management cycle method on the intervention of irrational medical orders of antibacterial drugs in the PIVAS of our hospital
焦点管理循环法对我院PIVAS抗菌药物不合理医嘱干预效果分析

摘要

目的 分析我院静脉用药调配中心（以下简称静配中心或PIVAS）应用焦点管理循环法干预抗菌药物医嘱合理性的效果。方法 采用焦点管理循环法（FOCUS-PDCA）对我院静配中心2023年（对照组）与 2024 年（观察组）抗菌药物医嘱进行回顾性评价，记录两组不合理医嘱信息信息、对比不合理医嘱数的比例。结果 对照组例数197例，涉及科室20个，涉及药品种类15例，有20例不合理医嘱类型，以配伍不合理、医嘱录入错误、用药途径不合理为主；观察组例数236例，涉及科室27个，涉及药品种类16例，总计有12例不合理医嘱类型，以用药频次不合理为主。观察组不合理医嘱数低于对照组（P＜0.05）。结论 焦点管理循环法可提高静配中心抗菌药物不合理医嘱干预效果，减少抗菌药物不合理医嘱数。

Abstract

Objective To analyze the effect of applying the FOCUS-PDCA method to intervene in the rationality of antibiotic orders in the intravenous admixture service (IVAS) center of our hospital.
Methods A retrospective evaluation was conducted on the antibiotic orders in the IVAS center of our hospital in 2023 (control group) and 2024 (observation group) using the FOCUS-PDCA method. Information on unreasonable orders in both groups was recorded, and the proportion of unreasonable orders was compared.
Results In the control group, there were 197 cases involving 20 departments and 15 types of drugs, with 20 types of unreasonable orders, mainly including irrational drug combinations, incorrect order entry, and irrational administration routes. In the observation group, there were 236 cases involving 27 departments and 16 types of drugs, with a total of 12 types of unreasonable orders, mainly including irrational administration frequencies. The number of unreasonable orders in the observation group was lower than that in the control group (P < 0.05).
Conclusion   The FOCUS-PDCA method can improve the intervention effect on unreasonable antibiotic orders in the IVAS center and reduce the number of such orders.
