两种新型心肺复苏(CPR)培训方法在住培生心肺复苏培训中效果比较:医学虚拟现实模拟与翻转学习

摘要

目的 比较医学虚拟现实模拟与翻转学习在住院医师规范化培训(住培生)心肺复苏(CPR)教学中的效果差异,探索优化培训策略的可行路径。方法 采用随机对照试验设计,纳入130名住培生,随机分为VR组(n=65)与翻转学习组(n=65)。VR组通过Meta Quest 2设备进行多场景沉浸式训练,翻转学习组采用“课前自主学习+课堂病例讨论”模式。主要结局指标包括操作规范性(按压深度、频率、完全释放比例)和知识保留率(培训后1个月理论考核),次要指标涵盖临床决策能力与学员满意度。数据通过智能模拟人系统、标准化考试及质性访谈收集,采用SPSS 26.0与NVivo 12进行定量与质性分析。结果 VR组在操作规范性上显著优于翻转学习组(按压深度达标率:89.9% vs. 71.4%,p<0.01;频率达标率:91.0% vs. 78.3%,p<0.01)。翻转学习组知识保留率更高(理论考核85.8 vs. 73.3,p<0.01),且临床决策时间更短(43.1秒vs. 57.2秒,p<0.01)。结论 VR-CPR适用于操作技能标准化训练,而翻转学习更利于知识深度内化与决策能力提升。未来应结合二者优势,设计分阶段混合教学模式,以实现CPR培训的全面能力提升。

Abstract

Objective To compare the effectiveness of medical virtual reality simulation (VR-CPR) and flipped learning in cardiopulmonary resuscitation (CPR) training for resident trainees, and to explore optimized training strategies.
Methods A randomized controlled trial (RCT) was conducted with 130 resident trainees, randomly assigned to VR group (n=65) or flipped learning group (n=65). The VR group underwent immersive multiscenario training using Meta Quest 2 devices, while the flipped learning group adopted a “preclass selfstudy + inclass case discussion” model. Primary outcomes included operational compliance (compression depth, rate, and full recoil ratio) and knowledge retention (theoretical exam 1 month posttraining). Secondary outcomes covered clinical decisionmaking ability and learner satisfaction. Data were collected via smart manikins, standardized exams, and semistructured interviews, analyzed using SPSS 26.0 and NVivo 12.
Results The VR group outperformed the flipped learning group in operational compliance (attainment ratio in depth: 89.9% vs. 71.4%, p<0.01; attainment ratio in rate: 91.0% vs. 78.3%, p<0.01). The flipped learning group showed higher knowledge retention (exam score 85.8 vs. 73.3, p<0.01) and shorter decisionmaking time (43.1s vs. 57.2s, p<0.05).
Conclusion   VR-CPR is ideal for standardizing operational skills, while flipped learning enhances knowledge integration and decisionmaking. A hybrid model combining both approaches is recommended for comprehensive CPR competency development.
