Observation on the effect of integrated traditional Chinese and western medicine on the wound after anal fistula operation in the elderly and the healing time of the wound
中西医结合治疗老年肛瘘术后创面的效果及创口愈合时间观察

摘要

目的 将中西医结合方案，用于老年肛瘘手术患者，对此方案的治疗作用研究。方法 对象是70例老年肛瘘手术患者，研究时间2021年3月~2022年3月。35例术后进行头孢曲松抗炎治疗，命名为普通组；35例进行头孢曲松抗炎治疗的同时，辅以中药黄芪汤内服+中药熏洗治疗，命名为中西医结合组。研究临床病情控制优良率、创面疤痕面积、疼痛（NRS）评分、创面愈合时间、住院时间、不良反应发生率。结果 ①治疗2周后，中西医结合组的临床病情控制优良率94.28%，对比普通组74.29%而言更高（p＜0.05）。②治疗2周后，中西医结合组的创面疤痕面积、NRS评分分别是（1.89±0.14）cm2、（1.26±0.11）分，对比普通组（2.39±0.56）cm2、（2.85±0.28）分而言均更低（p＜0.05）。③中西医结合组的创面愈合时间、住院时间分别是（20.36±2.07）d、（16.45±1.36）d，对比普通组（25.39±3.17）d、（20.19±1.65）d而言均更短（p＜0.05）。④治疗2周后，中西医结合组不良反应发生率11.43%，对比普通组的8.58%，未显著增加（p＞0.05）。结论将中西医结合方案，用于老年肛瘘手术患者，能够获得更佳疗效，利于创面快速愈合，在减轻患者疼痛，缩小创面瘢痕面积方面也显示出重要治疗价值。

Abstract

Objective: To study the therapeutic effect of the combination of traditional Chinese and western medicine on elderly patients undergoing anal fistula surgery.
Methods: 70 elderly patients with anal fistula were enrolled in the study from March 2021 to March 2022. 35 cases were treated with ceftriaxone for anti inflammation after operation, named as the common group; 35 cases were treated with ceftriaxone and huangqi decoction plus fumigation and washing of traditional chinese medicine. The rate of clinical condition control, wound scar area, pain (NRS) score, wound healing time, length of stay, and incidence of adverse reactions were studied.
Results: ① after 2 weeks of treatment, the excellent and good rate of clinical condition control in the integrated traditional chinese and western medicine group was 94.28%, which was higher than that in the ordinary group (74.29%, p<0.05). ② After 2 weeks of treatment, the wound scar area and NRS score of the integrated traditional Chinese and western medicine group were (1.89 ± 0.14) cm2 and (1.26 ± 0.11) points respectively, which were lower than those of the ordinary group (2.39 ± 0.56) cm2 and (2.85 ± 0.28) points (p<0.05). ③ The wound healing time and hospitalization time of the integrated traditional Chinese and western medicine group were (20.36 ± 2.07) d and (16.45 ± 1.36) d respectively, which were shorter than those of the ordinary group (25.39 ± 3.17) d and (20.19 ± 1.65) d (p<0.05). ④ After 2 weeks of treatment, the incidence of adverse reactions in the integrated traditional Chinese and western medicine group was 11.43%, which was not significantly increased compared with 8.58% in the ordinary group (p>0.05).
Conclusion  : The combination of traditional Chinese and western medicine for elderly patients with anal fistula surgery can achieve better curative effect, which is conducive to rapid wound healing. It also shows important therapeutic value in reducing patients' pain and reducing wound scar area.
